Acknowledgment of Insurance, Liability Waiver & Permission to Treat
I confirm that the student is physically fit to participate and has had a physical exam within the last 12 months.
I give permission for myself and/or my child(ren) to take part in classes, events, and activities at UPPER Jiu Jitsu Academy. I understand that I am responsible for carrying my own accident and medical insurance.
In case of injury or emergency, I authorize standard medical treatment and transportation at my own expense or that of my insurance provider.
I understand that martial arts involves physical contact and includes risks of injury, paralysis, or death. I accept all risks—known or unknown—even if caused by negligence of UPPER Jiu Jitsu Academy, its staff, owners, or agents.
I release and hold harmless UPPER Jiu Jitsu Academy, its instructors, owners, employees, and representatives from all liability or claims related to participation. I agree to follow all safety rules and instructor directions and will notify staff if I observe any hazard.
I further release UPPER Jiu Jitsu Academy, its affiliates, sponsors, and facility owners from all liability for injury, disability, death, or property loss.
Membership & Cancellation Policy

UPPER Jiu Jitsu Academy may update policies, schedules, forms, or curriculum at any time.

Classes may be canceled or rescheduled if fewer than four (4) students attend.

Progress in training is not guaranteed and depends on attendance, effort, and instructor evaluation.

All memberships and subscription products are non-refundable.

All memberships include a 90-day minimum commitment.

After 90 days, members may cancel with a 30-day written notice submitted via the official cancellation form.

Early termination within the initial 90 days will result in the remaining balance being charged and the reversal of any discounts, including uniform discounts.
Photo & Media Consent
I authorize UPPER Jiu Jitsu Academy to use photos or videos of me/my child(ren) for promotional purposes, including the website, social media, and printed materials.
Acknowledgment & Acceptance
I have read, understand, and voluntarily agree to all terms above. I acknowledge that by signing, I am waiving certain legal rights without pressure or coercion.

At Upper Jiu Jitsu Academy, we are committed to providing a safe, respectful, and professional training environment for all students, coaches, and families.
1. Core Values

2. Expectations for Students
All students must:
- Treat coaches, teammates, and staff with respect at all times
- Follow all instructions to ensure a safe training environment
- Train with control and avoid reckless or dangerous behavior
- Maintain personal hygiene and proper training attire
- Refrain from bullying, harassment, or inappropriate language
- Report any injuries or concerns to a coach immediately
3. Expectations for Coaches & Staff
All coaches and staff must:
- Act professionally and lead by example
- Provide safe, structured, and age-appropriate instruction
- Treat all students equally and with respect
- Maintain appropriate boundaries, especially with minors
- Avoid any form of harassment, discrimination, or favoritism
- Follow all safety policies, including supervision standards
- Address misconduct promptly and fairly
4. Parent/Guardian Expectations
Parents and guardians are expected to:
- Support a positive and respectful training environment
- Communicate respectfully with coaches and staff
- Ensure their child follows academy rules
- Avoid interfering with instruction during class
5. Safety & Conduct Standards
- Zero tolerance for bullying, harassment, discrimination, or abuse
- No dangerous or unsupervised techniques outside of instruction
- All members must follow gym rules and safety guidelines
- Concerns or incidents should be reported to staff immediately
6. Social Media & Communication
- Members and staff must communicate respectfully online
- No posting content that could harm the reputation of the academy or its members
- Coaches should maintain professional communication with students, especially minors
7. Disciplinary Action
Failure to follow this Code of Conduct may result in:
All disciplinary actions will be handled fairly and at the discretion of Upper Jiu Jitsu Academy.
